What You’ll Do

• Provide compassionate, hands-on care navigation and skilled nursing for clients in their homes
   
• Help older adults age gracefully and safely at home — avoiding unnecessary hospital visits
   
• Manage medications, coordinate care, and communicate closely with families
   
• Offer skilled nursing services such as IVs, wound care, blood draws, and catheter care
   
• Partner with leadership and medical teams to ensure clients receive exceptional care
   
• Go beyond checklists — truly know your patients and their stories

What You’ll Bring

• Active RN license (Required)
   
• BLS and First Aid Certification
   
• A warm, professional presence and a passion for patient-centered care
   
• Excellent communication, organization, and problem-solving skills
   
• Experience with older adults or care coordination a plus
   

Other Requirements

• Valid driver’s license & auto insurance
   
• Current CPR, annual TB test, and flu vaccine
   
• Clear background & drug screening
