💙 What You’ll Do

As a Home Health RN, you will serve as the clinical leader in the home setting, responsible for:

  • 🩺 Performing comprehensive patient assessments (physical, psychosocial, functional, and environmental)
  • 📋 Developing, implementing, and updating personalized plans of care
  • 🏡 Providing skilled nursing care for patients with complex and chronic conditions
  • 👨‍👩‍👧 Educating patients and families on self-care, medications, nutrition, and disease management
  • 🔄 Evaluating patient progress and adjusting care plans as needed
  • 📞 Communicating changes in patient status to physicians and interdisciplinary team members
  • 🦠 Ensuring strict adherence to infection control and safety standards
  • 🧑‍⚕️ Supervising and directing LPNs/LVNs and Home Health Aides to ensure safe, effective, and high-quality care delivery
  • 📊 Monitoring clinical documentation to ensure compliance with regulatory and payor requirements
  • 📈 Reviewing patient outcomes and ensuring continuity of care across the team

💡 What We’re Looking For

  • Active and unrestricted Registered Nurse (RN) license
  • Experience in home health, med-surg, or acute care preferred
  • Strong assessment, leadership, and documentation skills
  • Ability to supervise and delegate to LPNs/LVNs and care staff
  • Compassionate communicator with a patient-first mindset
  • Ability to work independently and manage multiple cases in the field
  • Current RN License

  • Valid Driver’s License

  • CPR Certification

  • Valid Auto Insurance

  • At least 2 years of Home Health RN experience preferred

  • HomeCare HomeBase experience preferred
