🎯 What You’ll Do in This Role:


As a Community Care RN with our PACE program, you’ll be at the heart of a mission-driven team providing both direct and indirect care to older adults in our community. While you'll work closely with the Nurse Manager or a designee for guidance, your advanced clinical skills and judgment will shine as you navigate and manage complex health situations with confidence and compassion.

Primary Responsibilities:

🩺 Assess & Plan Care

  • Complete thorough assessments and home visits for new and current participants.
  • Identify care needs, set priorities, and develop individualized care plans.
  • Collaborate with Physical Therapy and act as a backup Triage Nurse as needed.
  • Participate in team meetings and coordinate care with the full IDT.

💊 Deliver & Manage Care

  • Provide treatments like wound care, IV therapy, and tube feeding.
  • Manage medications—from enrollment through education and pharmacy coordination.
  • Monitor participant health, respond to emergencies, and document care accurately.
  • Support transitions of care, including hospital admissions and discharges.

🧭 Lead with Purpose

  • Attend daily huddles and support smooth clinic operations.
  • Delegate tasks appropriately and mentor team members.
  • Promote safety, participate in quality improvement, and champion best practices.

 What You’ll Need:

Required:

  • Graduate of an accredited School of Nursing
  • Active RN license in the State of Michigan
  • CPR certification or willingness to obtain
  • Valid Michigan driver’s license and access to reliable transportation

Preferred:

  • 3+ years of nursing experience in a health-related setting
  • At least 1 year of experience working with older adults or a frail population
