Patient Care Responsibilities Create and manage individualized patient care plans . Adjust the care plan based on how the patient responds to treatment. Evaluate effectiveness of treatment and outcomes. ‍⚕️ Care Coordination Coordinate and supervise patient care when required. Work with the healthcare team to ensure proper patient treatment. Organization & Prioritization Ability to organize workload and prioritize tasks efficiently during shifts. Assessment & Education Use patient assessment data to determine care needs. Provide patient education and discharge planning . ⚠️ Monitoring Patient Condition Identify abnormal findings during assessments. Report issues and update the care plan accordingly .
