Travel Nurse RN - Care Manager - $2,520 per week

LanceSoft is seeking a travel nurse RN Care Manager for a travel nursing job in Torrance, California. Job Description & Requirements Specialty: Care Manager Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Position Overview: We are seeking an experienced Registered Nurse (RN) Case Manager to support care coordination and discharge planning in an acute care facility. The RN will collaborate with physicians, patients, and interdisciplinary teams to ensure efficient patient care transitions and appropriate discharge planning. Assignment Details Start Date: ASAP Patient Ratio: 1:20 (Days) Facility Type: Acute Care Hospital Floating: Not required Weekend Requirement: Every other weekend Holiday Requirement: Must be available for holidays RTO: Maximum 7 days Key Responsibilities Coordinate discharge planning and patient care transitions . Collaborate with physicians, nurses, patients, and families to ensure appropriate care plans. Monitor patient progress and identify discharge needs early in the hospitalization . Facilitate referrals to post-acute services and community resources . Maintain accurate documentation in the electronic medical record . Ensure compliance with hospital policies and regulatory standards .
