Experience Requirements: • Two years of inpatient/acute care discharge planning experience • Hospital’s bed size experience should be 250-300 - notated on the jobs • Must be able to cover a variety of adult units with high acuity – ER experience is a plus • Epic Charting is required • Certs required: CCM preferred but not required.
