As a member of the Interdisciplinary Team, the Home Health Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to our frail elderly participants. The RN's responsibilities including nursing assessments, care planning, interventions, medication administration/management, treatments, nurse delegation and patient teaching. The Registered Nurse utilizes nursing judgment and documents findings in accordance with agency policies.
