Assesses, plans, implements, coordinates, monitors, and regulates compliance options and services to meet an individual's health needs throughout the continuum. Collaborates through communication and available resources to promote quality cost-effective outcomes. Expedites linking patient with systems in the community in conjunction with ED MSW to provide them with resources, services and opportunities to achieve and maintain maximum functioning. Facilitates concurrent documentation in the electronic medical record to accurately reflect care, services and resources utilized in the delivery of patient care. Successfully meets requirements for productivity and accuracy related to all utilization management processes.
